...The Royal's chief executive, William McKee, made a public apology after an inquest ruled that Mrs Murtagh's death had been due to a " systems failure" at the hospital.

An independent review, which was carried out by the Regulation and Quality Improvement Authority (RQIA), found there was confusion about access to emergency theatre and that medical staff "were not sufficiently sensitive to a patient whose condition was not improving".

The review made 11 recommendations and identified a number of areas of concern including consent, patient care, leadership and communication and the implementation of policies and procedures.
